The final Thirlwall Inquiry report into the Lucy Letby case has been released, revealing what happened inside the Countess of Chester Hospital as doctors became increasingly concerned about unexplained neonatal deaths. Lady Justice Kathryn Thirlwall concluded there was a “complete failure to protect babies” and that some deaths and attacks could have been prevented if hospital leaders had acted sooner.
